1. No-Fault Information Required: All patients are
required to bring at the time of your initial appointment the insurance
carrier name, address and phone number as well as the policy number
of the vehicle in which or by which you were injured. If you were injured
by a vehicle as a pedestrian or while riding a bicycle, please make
sure to have the vehicle policy number that hit you. Please also make
sure you have a claim number from the insurance carrier. All claims
must be put in to the insurance carrier within 45 days of the date of
accident.

Injuries on the Job: If you were injured on the job,
while working or on your employer’s property you are entitled
to worker’s compensation benefits. 3. No-Fault vs. Workers’ Comp: If you are driving a vehicle while working (i.e. delivery truck or taxicab), your claim is a worker’s compensation claim and should be filed with your employer’s workers’ compensation carrier. 4. Must Be Reported: All workers’ compensation claims must be reported to this office within 72 hours of treatment as our claims go out regularly. All claims are submitted directly to the carrier and to the state. 5. Authorization for treatment: All therapy services require authorization from the insurance carrier. Authorization can take up to 72 hours to receive but no longer than 30 days and is subject to medical necessity. 6. Disability Forms: All Disability forms are subject to a $25 or $50 fee by the physician depending on the length of the form. This fee is for the time involved in completing the information required on your form. 7. Multiple Services on Same Day: Worker’s Compensation patients cannot have services from multiple provider specialties on the same day. This includes other practitioners, chiropractors etc. 8. Attorney: If you have an attorney handling your case, please make sure to give us their name, address and phone number.
